I attest that I have completed the required Abuse, Neglect, and Exploitation (Duty to Report) Training for Beaton Care Service Inc. as a participating provider or staff member serving Molina Healthcare members, and that I have reviewed the Reporting of Suspected Abuse, Exploitation and/or Neglect provisions of the Molina Healthcare of Florida Medicaid Provider Handbook, 2026 edition, pages 104–106. I understand I am a mandatory reporter under Florida law and must immediately report known or suspected abuse, neglect, or exploitation to the Florida Abuse Hotline (1-800-96-ABUSE), and notify my supervisor and the Compliance Officer, Liannet Delgado, after any report.

I attest that I have completed the required Human Trafficking Awareness Training for Beaton Care Service Inc. and reviewed the Identifying Victims of Human Trafficking provisions of the Molina Healthcare of Florida Medicaid Provider Handbook, 2026 edition, page 107. I understand my obligation to report known or suspected human trafficking to the Florida Abuse Hotline (1-800-962-2873) or, in an emergency, 911, and to notify my supervisor and the Compliance Officer, Liannet Delgado, following any report.
